(a) Request the Sign-up Package.
(b) Complete and sign all documents.
(c) Include a check for $195 payable to Major Referral Network Company.
(d) Include your original license together with $25 application fee payable to
the Illinois Department of Financial and Professional Regulation (IDFPR).
(e) If you just pasted the state exam include your state examination paperwork together with the original school transcripts and $125 application fee payable to the Illinois Department of Financial and Professional Regulation (IDFPR).
